About Your Visit
Thank you for scheduling your appointment with Colorado Limb Consultants. Our physicians look forward to meeting with you. Please arrive 15–20 minutes prior to your appointed time to complete the registration process. If you have questions regarding any of this information, you may phone our staff prior to your appointment or you may address your questions in person during the pre-registration process.
We accept most major insurance plans. If your insurance company requires a referral to see a specialist, it is your responsibility to arrange for this in advance. If you are unable to obtain a referral, you will be asked to sign a waiver of liability form at the time of your appointment. It is also important that you bring your insurance card to your appointment.
Please call the office to make an appointment in advance. Please arrive 15–20 minutes prior to your appointed time to complete the registration process. Please complete these forms and bring them to your appointment. Any questions you have will be answered at that time. Please do not attempt to fax, mail or email your completed forms as this may delay the check in process. Please keep in mind that as a new patient, your appointment may last up to two hours.
If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Fees & Payments
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. If we are a participating provider of your insurance company, we will bill them. If a co-pay is owed for the visit in our office, payment is expected at the time of service, unless other arrangements are made in advance. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Prescriptions & Renewals
Please request all prescriptions and authorizations during our regular office hours, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances.
For Your Appointment
If you are a new patient, please arrive to your appointment 15–20 minutes early in order to complete the registration process. In addition, please bring the following:
- Driver's License or State ID
- Referral (If Required by Insurer)
- Insurance Information
- Copies of Operation Records, Medical Records, X-rays (films/cds/discs), MRIs, and CT Scans (films/cds/discs) and Pathology Slides (if applicable) from Prior Doctor Visits
- A List of Current Medications and Any Known Allergies
This information will eliminate delays and also help your physician establish a treatment plan.
